  I didn't have a good experience with Provo Health Department. I simply wanted a copy of my food permit sent to my address in a different city. I paid the fee, gave all the necessary information and I thought all was well. I waited about a week... no mail. I needed that food permit for my new place of employment, so I called the Health Department. My card was sent back! Now this is what bothers me.. they didn't tell me. Instead of notifying me so I could fix potential errors on what they put for the address, the sent it out again! And guess what!? It got sent back to them. Apparently there is a policy that if something is sent back twice, it must be picked up in person and can no longer be mailed. I attempted to correct THEIR mistake, but they wouldn't let me. When I called to work things out the Customer Service Rep started to yell and get extremely worked up. I am just trying to get a stupid food permit for heavens sake! And they weren't being very helpful or honest may I add. A dear friend was able to go in for me, and then send it to me in due time. I discovered upon receiving it that they had written down our address incorrectly! When I was talking with the Customer Service Rep I asked him to read out the address on the mail. He lied to me and read out the wrong address and simply repeated the address I had just told him. I am extremely disappointed with the service at the Provo Health Department and something needs to be done to fix it. I hate that I have to give them even one star before I can post this.

  DO NOT come here for your immigration physical exam. Physician is nice but slow and careless when filling out paperwork. He can mess up your paperwork which can cause huge delays in your permanent residency process. Staff also didn't care about him messing up the paperwork.
